Certificate of no criminal record

Insurance and maintenance included
Car not older than 2005

Necessary documents to obtain an identifier

Polish driver's license
Medical certificate for taxi driver
Psychological test for taxi driver
Certificate of no criminal record in Poland (not older than 30 days)
Certificate of no criminal record in your own country (issued no more than 30 days before submission to UŻGON)
Sworn translation into Polish of certificate of no criminal record from your own country
